Position Highlights:
Nebraska Medicine is now taking applications for a talented individual to join us as a Pharmacy Technician - Transitions of Care Senior Associate on our infectious diseases and outpatient parenteral antimicrobial therapy (ID/OPAT) pharmacy team, with responsibilities for both inpatients and outpatients. This role will be great for someone that is detail-oriented, patient-centered and enjoys working in a dynamic multidisciplinary care environment alongside pharmacists, nurses, and physicians. The ideal candidate will have a passion for patient care, customer service, and desire a fulfilling career focused on patients with complex antimicrobial regimens.
We seek experienced Pharmacy Technicians with a passion for Pharmacy and direct patient care. The staff within the Pharmacy Department at Nebraska Medicine play an integral part of healing and serving our patients. The team partners with our physicians, nurses, and other hospital staff to provide the best Serious Medicine and Extraordinary Care available in the region.
The primary work location for this position will be on main campus in a decentralized role.
The ideal candidate will have minimum of three years of experience as an inpatient or outpatient pharmacy technician or other applicable health care setting. You must already be certified as a Pharmacy Technician in the State of Nebraska.
The role will have the following primary responsibilities:
Performing medication histories and compliance assessments for ID/OPAT patients
Verifying accuracy of treatment plan communications during the transition of care process
Obtaining and entering laboratory results for ID/OPAT patients
Preparation, documentation, and execution of applications for pharmaceutical patient assistance
Assisting with training new staff, completing these activities and identifying opportunities to optimize the transition of care process for ID/OPAT patients
May be involved in transitions of care projects and research

Duties:
Facilitate patient care transitions at admission, transfer, and discharge from the facility. Utilize available electronic resources, patient interview, and phone follow- up to ensure accurate medication-related information is documented for patients across the continuum of care.. Work in coordination with and under the supervision of a registered pharmacist to review transfer orders and ensure they have been accurately communicated during the discharge process. Collaborate with ID/OPAT team pharmacists and nurses, pharmacy financial counselors, and discharge planning staff to assist in ensuring patient pre-discharge and post-discharge needs are met in a timely manner.
Required Qualifications:
Minimum of three years of experience in an inpatient or outpatient pharmacy or other applicable health care setting required.
High school diploma or equivalent required.
Effective communication skills, initiative, teamwork and flexibility required.
Registered as a pharmacy technician with the State of Nebraska Board of Pharmacy prior to start date required.
Pharmacy Technician Certification within twelve months of registration date required.
Basic Life Support certification within 90 days of transfer or hire required.
Preferred Qualifications:
Thorough understanding of available electronic resources to obtain medication history information preferred.
Thorough understanding of medical terminology preferred.
Thorough knowledge of top 100 medications preferred.
Associates or bachelor's degree preferred.
Proficient in Microsoft Office preferred
