Job Description:

We are seeking a Home Health Physical Therapist for a 13•week contract in Burnsville, MN. This role involves delivering personalized care to patients in their homes, with a 40•hour workweek.

Responsibilities:

• Provide quality physical therapy to home health patients.
• Document using Homecare Homebase (HCHB) and OASIS, with same•day submission.
• Manage a caseload of 20•25 cases weekly, achieving 26•28 productivity points.
• Attend weekly in•person office meetings.
• Offer flexible care to meet patient needs.

Requirements:

• Valid PT License and BLS Certification.
• 2+ years of home health experience with HCHB/OASIS proficiency.
• Experience with Oasis E post•January 2023.
• Consistent work history with minimal gaps.
• Reside within 30 minutes of the territory or arrange nearby housing.
